Registration form must be sent in via email or fax 48 hours in advance prior to each clinic date so conformation of attendance can be made to assure enough VA personnel can be in attendance.
(A signed copy will be required to be handed to VA personnel on the day of the clinic before participating or can be faxed 48 hours before clinic).
